Q What do I do if I want to quit or transfer to another program?
A Complete the drop transfer form and mail it into CCCC office Attn: Food Program.

Q Do I have to allow the Food Program Monitor in to do a site visit?
A Yes, even if you are not providing care the monitor must review records to assure they are up to date.

Q What kind of pen should I use?
A A black ink medium ballpoint pen.

Q May I use white out if I make a mistake?
A Yes, white out should be used if a mistake has been made.

Q What do I do if I lose my provider ID number?
A Contact the office, they will always have that information.

Q When filling out an enrollment form how do I know what number to give the child?
A Your Child Enrollment List, you assign the first available number.

Q When I drop a child from my program may I reassign the child’s number to a new child?
A No, please do not reuse that number until you’re given a new Child Enrollment List and the number is blank.

Q If I have to put in my provider number, why do I have to print my name?
A It is a safe guard to protect your claim. If you ever forget to put in your number and we can’t read your signature.

Q What happens if my paper work is not up to date on my site visit?
A The monitor must disallow any meals not properly documented which will result in the loss of reimbursement for those days.

Q How many visits will my monitor make to my home?
A Monitors are required to visit a minimum of three times a year. One visit must be unscheduled and unannounced. If you are having difficulty or your claim warrants it more frequent visits will occur.
